What Are Swing Bed Services?
A Swing Bed is an extended care service that Medicare has developed for a unique subset of patients. These patients no longer require acute hospitalization. However, they are not yet strong enough to care for themselves at home safely.
The Swing Bed program is designed to strengthen and empower these patients and help them regain independence. To achieve this, we work with patients one on one, encouraging them to do as many daily activities as possible. This includes dressing, bathing, ambulating, and so on. As they become successful in these tasks, we can be confident that they are ready to safely transition back home.
We ask all patients who come into our Swing Bed program to bring in clothing and personal care items from home. This will help them feel comfortable while working towards their goals of independence once more.

Swing Bed Coverage Information
Medicare pays 100% of the first 20 days of Swing Bed coverage. On the 21st day, Medicare will pay 80%. The patient or secondary insurance provider will be responsible for 20% of the bill. You are allotted 100 qualifying Swing Bed days in a calendar year.
The eligibility requirements include:
- Physician ordered
- Must have three consecutive days in acute care
- Services must begin with 30 days of acute stay
- Care needs and/or problems must warrant skilled care that our facility can provide
If you have any concerns about going home after a hospital stay, talk to the discharge planner regarding our Swing Bed program.
Understanding the Need for Skilled Care
Skilled care requires professional management as an in-patient. This care is needed 24/7. Examples can include:
- Monitoring and adjusting medications
- IV antibiotics
- Physical therapy (after knee surgery, hip surgery, or cardiac surgery)
- Wound care
- After COVID-19
- Cardiac and pulmonary rehab
To determine your need for skilled care, your case will be assessed by the Care Plan Team. They will consider if your needs are a result of a medical condition. They will also consider chronic conditions that could complicate your acute problem. Finally, they will look at complications that arise from your treatment.
Your Care Plan Team includes:
- Providers
- Swing Bed Coordinator
- Director of Nursing
- Dietary Staff
- Nursing Staff
- Social Services
- Patient and family
- Physical Therapy
- Respiratory Therapy
- Activities Staff
